Guidelines for Packing & Shipping Pharmaceutical Returns

Did you know that broken bottles or leakage of medications may make some of your product non-returnable? If the label is damaged (Lot #, Expiration date or NDC number not legible), manufacturers may not accept the product for return. Major leakage may even put the whole box in jeopardy.

Here are some guidelines to help you get your pharmaceutical returns to us safely & securely.

  1. Place all liquids in plastic bags to prevent leakage. Please cushion wrap (e.g., bubble wrap or cardboard) all glass bottles and place in a zip lock plastic bag for safe transport.
  2. Please do not stuff partially filled bottles with cotton or tissue.
  3. Please make sure all bottle caps are securely fastened.
  4. Please double box open or unused needles and auto inject pens if not in original case or package. Please note that we do not accept regulated medical waste including unused exposed needles.
  5. Please fill the void in all boxes with packing material or bubble wrap.
  6. Avoid using boxes over 18’x18’x18’; oversize charges may apply for prepaid shipping programs.
  7. Please use the Return Authorization mailing labels attached to the bottom of your Return Authorization form(RA) to mail your return to us.
  8. Please include a Return Authorization form in each box that you ship. If additional forms or labels are needed, please make photocopies.
  9. Please be sure that the following information is correct on your Return Authorization form. If this information is not correct, please call our Customer Service department at 1.800.473.2138.
    • Your Name, Address & Phone Number
    • Your Wholesalers Name, Address & Account Number
    • Your DEA Number
    • All Other Pertinent Information

Requirements for Controlled Products

Controlled pharmaceutical returns have some special shipping & handling instructions. Please follow all instructions to avoid any delays on your return.

CII Handling

Please do not ship Schedule II (CII) products until you receive your DEA 222 form from us.

Please complete the DEA 222 Request Form so that your DEA 222 form can be generated.

After your 222 form is generated, it will be sent to your facility with:

  • An RFID form
  • A Return Authorization (RA) form
  • A listing of DEA Field Offices.

DEA 222 Forms expire 60 days from the issue date so please ship your Schedule CII products immediately.

Please follow these instructions when mailing your CII products:

  1. Package all CII products as suggested above in helpful hints 1-9; then place all CII pharmaceutical products in a box. CIIs must be shipped in a separate box from all other pharmaceutical returns.
  2. Place the Return Authorization Form (RA) that was sent to you by the GRx Control department in the box you are shipping. If you are shipping more than one box, please make copies of the RA form and place a copy in each box you ship.
  3. Separate the DEA 222 form into its (2) pages.
  4. Complete the packages and date shipped columns on the Green page.
  5. Send the Green page to the DEA. Keep the Brown page for your records.
  6. Put a copy of the Brown page of the DEA 222 form in the box with your CIIs.
  7. Place the RFID form on top of your merchandise inside the box.
  8. Seal the box with packaging tape.
  9. Place a Return Authorization mailing label on the outside of each box. You may make photocopies of the original, if more labels are needed.
  10. FedEx shipping labels cannot be photocopied; each label must be an original.
  11. Mark the box “Box 1 of #”.
  12. Please complete the procedure for all additional boxes.

CIII-CV Special Instructions

Please separate all Schedule III – Schedule V (CIII-CV) pharmaceutical products from all other prescriptions, OTC, HBC and Medical-Surgical pharmaceutical products.

When you have less than a full box of Controls (CIII-CV), please follow these instructions:

  1. Package all CIII-CV products as suggested above in helpful hints 1-9; then place all CIII-CV pharmaceutical products in a plastic bag separate from your non-controls.
  2. Print a copy of the Return Authorization Form(RA) and list all the CIII-CVs that you are returning on the RA form.
  3. Place a copy of the RA form in the bag and seal each bag with packaging tape.
  4. Place a Return Authorization mailing label on the outside of each bag. You may make photocopies of the original, if more labels are needed.
  5. Place the labeled CIII-CV bag(s) in your returns box along with the non-controlled items.

When you have a full box of Controls (CIII-CV), please follow these instructions:

  1. Package all products as suggested above in helpful hints 1-9; then place all CIII-CV pharmaceutical products in the first box of your shipment.
  2. Print a copy of the Return Authorization form (RA) and list all the CIII-CVs that you are returning on the RA form.
  3. Indicate on the Return Authorization form “FULL CONTROL BOX”.
  4. Place a copy of the RA form in the box and seal the box with packaging tape.
  5. Place a Return Authorization mailing label on the outside of each box. You may make photocopies of the original, if more labels are needed.
  6. FedEx shipping labels cannot be photocopied; each label must be an original.
  7. Mark the box “Box 1 of #”.
  8. Please complete the procedure for all additional boxes.
